Admission Requirements

SENIOR HIGH SCHOOL GRADUATES

Students promoted to Senior High School and wish to enroll at AMYA POLYTECHNIC COLLEGE, INC. are required to submit the following:

  1. Form 138 (Report Card)
  2. Honorable Dismissal/Certificate of Transfer Credentials
  3. Certificate of Good Health issued by AMYAPCI School Physician (with laboratory results from an accredited hospital). Hepatitis Screening is required for all Health-Related programs.
  4. Certificate of Good Moral Character from the previous school attended.
  5. Birth Certificate on Security Paper from the Philippine Statistics Authority (PSA) (original and photocopy)
  6. Two (2) copies of recently taken 2x2 colored pictures.
  7. A photocopy of marriage contract on Security Paper from the PSA (for married students).

TRANSFEREES/GRADUATES FROM OTHER DISCIPLINES

Transferees are students previously enrolled in other schools who wish to enroll in AMYAPCI. Graduates from other disciplines are those who have finished a degree and wish to enroll in AMYAPCI. are required to provide the following:

  1. Photocopy of Transcript of Records for evaluation and reference.
  2. Honorable Dismissal/Certificate of Transfer Credentials.
  3. Certificate of Good Health issued by AMYAPCI School Physician (with laboratory results from an accredited hospital). Hepatitis Screening is required for all health-related programs.
  4. Certificate of Good Moral Character from the last school attended.
  5. Birth Certificate on Security Paper from the Philippine Statistics Authority (original and photocopy).
  6. Application for Admissions with Pledge of Agreement (for incoming 4th year students).
  7. Two (2) copies of recently taken 2x2 colored pictures.
  8. A photocopy of marriage contract on Security Paper from the PSA (for married students).

SHIFTEES

A shiftee is any student who is currently enrolled in AMYAPCI and wishes to shift from one course to another program considering that the student must accomplish and submit the following requirements to the accepting program as it follows:

  1. Complete records of the student grade ledgers.
  2. Accomplished Application Shift Form.
  3. Pass the interview of the accepting Program Chair.

RETURNEES

Previously enrolled at AMYAPCI and subsequently enrolled in other schools:

  1. A request letter for readmission that is subject to the Program Chair’s approval.
  2. Transfer of the school credentials from the last school attended (Honorable Dismissal, Transcript of Record, Certificate of Good Moral Character).
